This is a 2-Part process:

1. Former Students/Alumni: To order your transcript, ACT scores, or shot record, complete this MJHS Transcript & Information Request Form.
2.  PAY for the requested records, go to Online Payments 

**Please allow at least 1-2 weeks for this request to be processed by the post-secondary institution.**
